La Vita Care supports people that have had an injury or illness, have mobility issues or disabilities, or those that just need some additional support to live independently in their own homes.

We know that life happens. A fall, the loss of a partner, a recent diagnosis or sudden loss of mobility. Whether you need help during your recovery from a recent hospital visit or you are looking for longer term support, we’re here to assist you.

While you might be experiencing some challenges in your overall ability, we take a person-centered approach. That means we’ll focus on your strengths and what you can do by yourself safely, then build support teams around you to amplify your existing strengths.  We seek to support you to improve your independence and your quality of life.  Our approach means we want to keep you as independent as possible and as active in your community as you choose to be. Life on your terms, with the support you need to do the things you love.

In-Home Care

In-Home Care includes personal care and help with daily tasks such as dressing and showering. We can also assist with light housekeeping duties and other daily tasks. Home Support can go a long way in helping you maintain your independence.

Supported Independent Living

Supported Independent Living is for people with higher support needs, who need some level of help at home all the time. Supported Independent Living (SIL) is best suited to people who need a significant amount of help throughout the day, 7 days a week, including overnight care.

Community Access

You may need some help getting to a medical appointment, or prefer to be escorted on a shopping trip, to help manage bags or get in and out of vehicles. Getting around safely might mean having a companion to support you. Whether you want to visit your local social club, see a movie or visit a gym or aquatic centre, La Vita Care are there to help. We want to continue to support you to do the things you love. We can help with transport and provide companions to help keep you active and connected to your community.

Hospital Transition

We understand that rehabilitation after an accident or hospital visit can be a particularly difficult time.  We provide personal care and attendant care services, respite and overnight care, light rehabilitation support services including assisting with rehab exercises and helping with mobility aids.  Our services may also include skill development such as helping with basic meal preparation or supporting you to manage your medications.
